Regarding images made by other artists, the most friendly interpretation would be that the shop owner would have bought an extended license that allows images to be used on print on demand products like on Zazzle.
Model releases are not checked by Zazzle, so it is the responsibility of each shopkeeper to take care of it or to get sued by the model in question. Hope that helps, SY
PS If you want to join the fan club of somebody that is not on the same .tld like you, just change it for yours. So for example if you see
http://www.zazzle.com/hospitalera* but your are on co.uk, just change it to
http://www.zazzle.co.uk/hospitalera*
