can I use my own model release forms?

Hi,
I have photos that were released to me using a generic model release form (no mention of SS, no Witness signature and no Description of Scene!) Just wondering if I can use them or if I have to try and hunt down the models and get them to resign a new release.

Also, since SS allows photographers to submit the same content onto other stock sites, what kind of release do you use so that the model only has to sign one form that can be used on multi sites?

You can use your own generic form so long as it contains all the pertinate information that is on the SS release. This must include a witness name and signature. The generic form can usually be used at most sites. Note that one site (IS) has recently instituted a rule that each and every model shoot must have its own release. You can no longer use the same release for multiple shoots. That's not the case, presently, at SS.

No. The witness is meant to watch the model sign the release. They don't need to see the actual shoot, just confirm that it was the model who actually signed the release.
