So this is the only form I need to fax in, correct?
Do I need to fax in all 4 pages? Or can I just do page 1, considering it's the only page I fill out. |

From what I understand and what I have ever sent in is the page that you fill out and that is it. |

Ok I faxed it in. Is there any way to know if was received? Other than that pesky email stopping? |

I just faxed the first page and it was received successfully after about 3 or 4 business days. |

yes, i got an email confirmation; but that may have been only because i sent an email inquiry.
From your submit.shutterstock.com home, one way to tell that your W9 has been processed is that the red alert message at the top (notifying you to submit your W9) will go away. |
