Minor model release - huh?

I am a bit confused by this form.

In the section at the bottom, whose name do I put next to where it says 'Name (Print)', mine as the parent or the minor?

If mine, then why does it ask for the signature here and then for the signature of the parent/guardian later on? Surely there is only one signature needed and that is me as a parent so why two spaces for a signature? My 8 week-old doesn't have a signature!!

Am I missing something here?

And while we're at it, why say 'Signature of Parent/Guardian (if minor)'.

If they weren't a minor we wouldn't need the minor release form.

I always have mom or dad print childs name in all spaces and in parenthesis have them put (Name, parent) in every field that referrss to modles name, then have them sign the release.

Too funny I went through the same thoughts, sort of, as I was filling one out for photos of my son. If you pretend the child isn't the child of the photographer submitting the photo (i.e. your own child), it makes sense - even if that sentence I just wrote didn't.

Let's try again.

If you were to hand the form to another parent to sign for release of their child's photo you took - the form would make sense. Where you are both the photographer and the parent it kind of reads weird.

At least that was my impression when i did one for the first time.

I have a question about the general adult release and figured I would ask here rather than start a new thread.

If you are taking self portriats, can you be the witness? After all, you witnessed yourself taking the image of yourself. In other cases, who qualifies as a witness? It might just be you and the model.
