Example copy right release required decorative pattern?

Hi,
I would like to take a photo of a cup and saucer which I believe has a distinctive copy righted pattern on it. I have contacted the owner who is happy to release the copy right to me for use in commercial photographs (only).

1) Does anyone have an example Copy Right release form for this? Or can I use the Property Release form (eg. this form is not just for buildings/architecture)?

2) Does shutterstock accept Copy Right/Property releases for patterns on items like cups, china etc?

3) Does any of this make sense? :-) (I hope so)

It is not the owner but the creator / manufacturer who needs to give permission. A standard property release is ok.

It is the manufacturer of the cup and saucer, so that should be all good then.
Excellent, Many Thanks. I just did not want to be spending hours taking the shots only to have them rejected.
