Hello,

I have problem in filling this form of MR.
Please explain me.
Thank you in advance for all response.

From attachment:
No (1) in the attachment. Should I fill in the minor or the mother/father/guardian name?
No (2) in the attachment. Should I fill in the minor or the mother/father/guardian name?
No (3) in the attachment. Should I fill in the minor or the mother/father/guardian informations?

I would say
#1 is minor name
#2 is minor name
#3 is minor info

I have filled in as you suggested, but it was rejected because of MR.

I don't understand and asked via submit ask about a month ago, but no any reply return.

Not sure why it was rejected if you filled it in that way. Up in the top left portion (#1) that is the minor's name but make sure that the parent or guardian circles their relationship to the model whose name is printed on the bottom left side as well.

The information on the top right is the model's information and make sure the parent or guardian signs on the right side where indicated.

Yes I circled her mother and filled in her mother name on the left side; also had her mother sign and ID photo on the right side.

May I give you more information, I sent the image of a baby girl and her mother together in that image. I have attached the Minor MR and her mother MR for that image. That image was rejected due to MR, that I don't understand.

I can't answer it then. Were you sure to have someone other than the mother (who was in the image) sign as a witness? The model cannot sign as a witness to their own image and you cannot sign as witness and as the photographer as well. The witness is not signing to witnessing the photo shoot but rather the signing of the release.

No others in that picture.

I have attached the image and MR of baby and her mother for you to see in this reply too, please look at it.

The reject reason from inspector is:
Not Approved:
Please only submit model releases individually.

Ok, I think I see the problem. It looks like you scanned or photographed the two model releases in one scan or image and submitted the two as one. You need to submit two separate releases individually, one for the baby and the other one for the mother.

Agree. Only one agency that I am aware of requires all releases to be combined into a single image (IS). Everyone else wants them separate.

Thnk you very much hhltdave5 and Triceratops.

But how can I attach (upload)2 file in the 1 picture?
I ever try attach two adult MR,first add one MR and again another one. But they show only one MR, and the SS ask for another one. That is why I gather the two MR in the one image.

How can I correct this problem while add MR of 2 persons in the same image?
