I am new to shutterstock and stock photography arena. I want to upload a model's photo for the first time.
I have some questions regarding model release.
1. Into the model release form there are filled for photographer, model and witness name. I am little bit confused about “Name(Print)”. Should I have to print the name every time before filling the form? Or I can added the name digitally after scanning the filled up form(because for outdoor photography I may not know the model name).
2. Whether model release is required for editorial pictures? Should I select only “editorial” category for editorial photographs? What is the royalty commission structure?

Model releases are not needed for editorial images but they must follow Shutterstock's requirements for an editorial image. They must be newsworthy and be titled according to Shutterstock's specifications. If you go to Shutterbuzz you will find sections on editorial as well as information on releases.

As for the name all information put on the release must be done by hand and cannot be done digitally. The witness section is not someone who actually saw the photo shoot but witnessed the signing of the release.

Into the model release form why "Print" is written within brackets just after name field? What is exactly meaning of "Print"?
is there any link where filled up model release form can be seen?

Printing is simply putting down the name with block letters and not connected like you do with a signature.
Commissions for editorial are the same it is just that the image can only be used in certain ways and not for commercial selling purposes.
Seeing that releases have some personal information about a model most of us will not show a real release filled out but I am attaching the one that I use showing what goes where.
